Preventable deaths in Estonia stand at 1.5 times higher than the EU average, largely due to poor health behaviours and low medication adherence. Growing treatment costs of cardiovascular and other chronic diseases are placing a heavy burden on the Estonian health budget.
To address these critical issues, I62, in collaboration with leading general practitioners, developed eHealthPlan—a digital tool changing how Estonia manages chronic diseases and preventive care.
eHealthPlan enables a collaborative process where patients and their GPs set personalized 12-month health plans, reviewed quarterly and updated annually. Accessible via mobile devices, patients have their health plan "in their pocket," making it easy to report progress and communicate with their doctors. Physicians benefit from a comprehensive dashboard that tracks each patient's medical status, physical activity, medication adherence, and upcoming tasks. The treatment plans encompass setting health goals, medical targets like controlling high blood pressure, and lifestyle changes such as increased physical activity, improved sleep patterns, and dietary adjustments.
Importantly, eHealthPlan addresses the issue of low medication adherence by allowing doctors to monitor prescription fulfilments. Piloted among hundreds of patients, the tool achieved remarkable success: 87% of patients with high blood pressure reached their control targets within 12 months (compared to 44% of patients receiving ‘offline’ treatment). The validated exercise prescription feature connects users with independent physical exercise counsellors and family doctors, offering a scalable solution ready for expansion into neighbouring markets.
